résumé L'équipe mobile de gériatrie est une unité fonctionnelle hospitalière, pluridisciplinaire comprenant au minimum médecin, assistante sociale, infirmière d'évaluation, secrétaire. Elle intervient sur signalement dans les services de spécialités ou d'urgences auprès de patients âgés poly-pathologiques ou fragiles. Elle assure une évaluation au lit du malade, formule des recommandations et assure une coordination intra-hospitalière vers le plateau technique gériatrique de court séjour, services de soins de suites ou communautaire, (médecin traitant, services de soins à domicile). Par son intervention, elle prépare le retour à domicile ou le passage en institution en collaboration avec les services en charge du malade. Elle utilise l'évaluation gériatrique comme base de son intervention en utilisant des instruments validés et adaptés au terrain d'intervention. On distingue trois sites d'interventions préférentiels : les unités d'hospitalisation de courte durée pour l'aide à la gestion des urgences médico-chirurgicales et l'organisation de retour à domicile précoce, les services de spécialités médicales et chirurgicales pour les consultations avancées et le traitement des "bed-blockers" et l'action communautaire dans une activité de coordination et liaison notamment pour la régulation vers l'hôpital des situations de crise médico-sociale. Interface naturelle entre l'hôpital et la ville, les équipes mobiles de gériatrie contribuent naturellement à l'animation des CLICs et au développement des réseaux gérontologique ville-hôpital. &SUMMARY Mobile geriatric team is a multidisciplinary and specialized hospital unit including a physician, a research nurse, a social worker and a secretary. Intervention is based on screening for frail elderly patients staying in different specialized units like emergency department or medical and surgical wards. It comprises of comprehensive geriatric assessment as a standardized method of care using validated and adapted tools, which depends on the place of intervention. It makes recommendations for care and ensures coordination between the short-term and rehabilitation care and the community services. This kind of intervention prepares the return of the patient to his or her residence or long-term care homes in collaboration with the services responsible for the patient. Three different interventions are important in this regard. The first intervention gives assistance to the management of the medical and surgical emergencies within the emergency department and especially works on the early discharge of the patient from the hospital. The second intervention involves medical and surgical specialities for advanced consultations and management of the "bed-occupiers". The third intervention concerns a community action including coordination and proper guidance of the medico-social crises towards the hospital. Acting as a smooth interface between the hospital and the community, the mobile geriatric teams naturally contribute to the development of geriatrics networks.
